Welcome to the Cleveland Polytrauma Network Site

One of 23 specialized rehabilitation programs designed to coordinate post-acute care within VISN 10 for Veterans and active duty Service Members who have experienced Polytrauma (multiple injuries to more than one body part or organ system) and Traumatic Brain Injury (TBI) related to combat or civilian activity. Post 9/11 Military to VA (M2VA) case managers are available to assist Service Members transitioning from active duty to civilian life. M2VA Case Managers work in collaboration with DoD to establish VA health care, coordinate services and navigate community resources as needed.
