Support Clinic Teams

Currently there are 87 Polytrauma Support Clinic Teams (PSCT) located in VA medical centers across the country. The PSCTs provide specialized outpatient care. In coordination with their VISN PNS, PSCTs offer continued medical and rehabilitation care and support closer to your home community. PSCT programs may also be the entry point for outpatient rehabilitation services for those who have experienced a mild-moderate TBI or polytraumatic injury.

PSCT interdisciplinary rehabilitation teams include:

  • Physiatrist (rehabilitation doctor)
  • Social Work Case Manager
  • Nurse Case Manager
  • Physical Therapist
  • Occupational Therapist
  • Rehabilitation Nursing
  • Speech and Language Pathologist
  • Rehabilitation Psychologist
  • Other consultative services as needed

PSCT teams work with you to:

  • Manage and coordinate your rehabilitation plan of care
  • Provide treatment
  • Assist with integration back to your home and community
  • Identify community resources for care
  • Provide regular clinical case management and family support
