Central Virginia Health Care System Headache Center of Excellence

The Headache Center of Excellence (HCoE) treats Veterans with a history of traumatic brain injury (TBI) or refractory headaches. Treatments options are dependent on the individual needs of the Veteran. This can include medication and non-medication options such as cognitive behavioral therapy, physical modalities, neuromodulation devices and interventions used with refractory headaches to implement health promotion and prevention practices.

Clinical Services Offered

The headache center, which is located within the neurology department, offers appointments by Face 2 Face, Clinical Video Telehealth (CVT), VA Video Connect (VVC) and e-consults. The Neurology headache provider completes a detailed intake to accurately diagnose, appropriately treat and effectively communicate a treatment plan that is tailored to the individual Veteran’s needs. Capabilities in the headache center include administration of nerve blocks, botulinum toxin, acupuncture, and neuro modulation devices, along with medication options as indicated and strategies to improve contributing lifestyle issues. In addition, the headache center health psychologist offers cognitive behavioral therapy (CBT) for headache management, the social worker can help with case coordination and a chiropractor is available for those Veterans who have pain radiating from the neck that is contributing to their headaches.

Additional clinics that the headache center utilizes for help in addressing the needs of the Veteran with headaches include: Polytrauma/TBI, Physical Therapy, Interventional pain, Veterans Integrated Pain Center (VIP), and Whole Health and psychology. These clinics provide acupuncture, chiropractic care, relaxation training, stress management, mindfulness meditation, hypnosis, biofeedback, and psychoeducation.

Referral:

Veterans can be referred to the headache center through neurology or other specialty clinic, when there is a history of post traumatic headache or a refractory headache of greater than one year.

  • Veterans Headache Management class is presented once per month in a virtual platform.
  • Headache Evaluation and Management for Primary Care Webinar Series (professional).
  • Headache presentations for staff and residents on an as needed basis, to include webinars to reach all within VISN6.
Innovation

  • Ketamine Infusion for intractable headaches is offered in partnership with the anesthesia department
  • Headache behavioral health group(s) offered

In the coming months, health psychology will offer groups on headache self-management, reviewing strategies for managing contributing factors like stress, sleep, and diet.
